What are your fees?

Do you have a membership option? 

Although we do not have a 'membership' option, we do allow our patients to participate in an interest-free payment plan of $200/month until their balance is paid.

This works much like a membership, but instead of having a set number of visits that you feel you need to 'use or lose,' you and Dr. Coller can customize your visits to your individual needs. 

If you are interested in this option, you may indicate it on the "Pre-visit Questionnaire" that you fill out when you schedule your appointment. 

What is your payment plan option? 

To spread out the cost of visits, we offer our patients an interest-free payment plan of $200/month until their balance is paid. 

If you are interested in this option, you may indicate it on the "Pre-visit Questionnaire" that you fill out when you schedule your appointment. 

If you select this, your card on file will be charged $200 after completing your visit, and recurrent $200 payments (or whatever your remaining balance is) will be charged to your card until your balance is cleared. 

I'm a former patient of Dr. Coller's. Does that change anything? 

First of all, thank you for seeking me out! The hardest part of leaving a practice is losing touch with patients that I've come to care deeply about. I am honored that you have sought me out, and it would be a privilege to work with you here at Precision. 

If you are a patient that I have seen within the last year, then we will likely be able to hold a 40-minute follow-up visit instead of my usual 60-minute initial. Please request the 40-minute follow-up when you schedule, and unless there is a pressing reason that I do not think it is appropriate, I will authorize it. 

Additionally, to make the most of our shorter visit time, please download any recent labs and progress notes from our previous encounters, and then upload them to the Precision/Charm portal. 

Do you participate with insurance?

Since we prioritize what you need above what 3rd parties are willing to pay for, we are a cash-based practice. We do not participate with any insurance plans, including Medicare or Medicaid. 

With private or commercial insurance plans, patients may receive out-of-network reimbursement for our services if they submit their superbill/invoice to their insurer. However, since Dr. Coller is not enrolled in Medicare or Medicaid, government plans such as Medicare and Medicaid cannot be submitted for reimbursement

Ultimately, since each plan is unique to the patient, we cannot ensure reimbursement from any insurance plan.

Sometimes insurance can be billed for certain labs and other tests. Specialty tests are often not covered. 

Can I use my HSA or FSA? 

Yes. As a physician, Dr. Coller's services should qualify you to be reimbursed with your FSA or HSA funds. Supplements and testing can sometimes qualify as well. If your program requires a letter of medical necessity, we will be happy to supply that for you. 

Do you offer any referral incentives? 

Yes! If you are a current patient and you refer a friend or family member, both of you will receive a $50 credit! Win-Win!

What is Functional Health Professionals? 

Precision Functional Medicine partners with Functional Health Professionals, LLC to provide Care Manager support for its patients. These Care Managers assist Dr. Coller's patients with a variety of things including understanding test pricing, troubleshooting lab and pharmacy issues, and helping patients have a smoother all-around patient experience. 

What is Functional Medicine?

Functional medicine is an approach to healthcare that seeks to restore the proper functioning of the body by removing obstacles to health, replacing what the body needs, and balancing physiological pathways where necessary. Ideas like "root-cause resolution" communicate that functional practitioners "go deeper" into their patients' health history and attempt to incorporate more comprehensive data analysis. While traditional medical approaches use medications and procedures to stop something bad from happening, functional medicine looks for ways to promote health.

What sort of patients do you work with?

We work with all patients. As a family medicine physician, Dr. Coller is trained to provide care for all age groups, men, women, and children. 

What conditions do you specialize in?

Dr. Coller has had years of experience treating both acute and chronic illnesses. He has worked in the family practice setting, as well as in urgent care - online and in person. These experiences have provided a broad knowledge base for him to use as he cares for his patients. Even if he is not equipped to address an issue himself, Dr. Coller can guide his patients to where they can best receive the care they need.

As a functionally trained doctor, Dr. Coller has helped many patients with chronic conditions such as diabetes, heart disease, chronic fatigue, autoimmune issues, irritable bowel syndrome, SIBO, and other GI problems. He balances hormones, restores nutrient deficiencies, optimizes detox pathways, and more. 

Truly, the functional medicine approach provides a framework for empowering each person's body to find a path to healing and wellness. The 'disease' or 'diagnosis' that people carry is often simply the normal physiologic response to the underlying imbalances. When we correct those imbalances, the 'diseases' often resolve or go into remission. 

Can you provide an interpretation of functional medicine/specialty tests that I have done outside of your office? 

Yes, of course! I am happy to incorporate the data that you have already collected into your treatment plan. 

An important thing to keep in mind, however, is that initial visits are not intended for extensive test review/interpretation

Although there may be time for reviewing some test results, first visits are a critical opportunity for Dr. Coller to get to know you and to do a proper intake & interview. As we say, "functional medicine physicians treat people, not test results." Therefore, it is important to understand that a proper interpretation and treatment plan can only be achieved after having taken the time to thoroughly understand your story. The results make much more sense in the context of your health history and timeline. 

Are you a primary care office?

"Primary Care" is a poorly defined phrase that requires some clarification. 

Like other primary care offices, Precision Functional Medicine can be a 'first-line access point' into the medical system. We provide direct patient care, order labs, send prescriptions, provide referrals to specialists, and help you make sense of your overall health journey. 

As a mostly telemedicine practice, however, we do not offer things such as immunizations, in-house labs/ testing/imaging, screening exams, or procedures. We do not provide urgent or emergency care. 

Do you do in-office testing?

No. But we can provide the necessary lab requisitions and referral forms so that you can get your testing at your local facilities. Many of the 'specialty' or functional tests that we recommend can be drop-shipped to your home. You may then complete the test at home and mail it back to the lab directly. This provides an added convenience that many patients appreciate. 

Do you do IV therapies?

No. We do not provide IV or injection treatments at this time. 

How do I schedule an appointment?

Do you have a phone number that I can call?

You may call 616-330-1440 and leave Dr. Coller a message. He or one of his staff will return your call as soon as possible. If you are an existing patient, please consider sending a message through the secure patient portal. 

How can I message Dr. Coller? 

Existing patients can send a message through the secure patient portal. If you are not a patient, please use the Contact form on this website. 

Are there any rules/guidelines around messaging?

Messaging is a great way to communicate with Dr. Coller for relatively simple questions or clarifications. It is not the proper place to address new or complicated issues. If Dr. Coller thinks that you would be better served holding an appointment, he may encourage you to schedule a visit. 

Do you prescribe controlled medications? 

We do not prescribe controlled medications at Precision. 

What is your cancellation/no-show policy?

Please provide at least 24 hour notice for any cancelations/reschedules. Cancelations within 24 hours will be charged a $100 fee. 

How do I access my health record?

If you are an existing patient, please click on the Portal link and use your login credentials to access your record.
